NCH’s Brookdale Center For Healthy Aging & Rehabilitation Collaborates With Jewish Family & Community Services on Caregiver Outreach

April 24th, 2013 - Starting in May, families at The Brookdale Center for Healthy Aging & Rehabilitation will benefit from an expansion of programming due to a new collaborative outreach with Jewish Family & Community Services, a non-sectarian human service agency serving Southwest Florida. “As a rehabilitation hospital, caregivers play an essential role in our ability to achieve great outcomes for the patients we serve,” said Heather Baker, administrative director of The Brookdale Center.

Whether patients are recovering from a new condition such as stroke, or regaining strength from a debilitating injury—caregivers have to be well equipped to help patients make successful transitions to their homes. This involves not only setting up the home front, but also learning how to take care of themselves. New caregiver classes will help to address both.

“The Board of Jewish Family & Community Services is supportive of our effort to partner with community providers in meeting the needs of our community,” said Dr. Jaclynn Faffer, JFCS President and CEO. “By reaching out to the families at The Brookdale Center, our licensed clinical social worker will be able to meet people at their point of need—during a time of transition.”

The program will have both educational & supportive components, and is based on input obtained from people who have been in a caregiver role. The focus is to empower the new caregiver as he, or she takes on new responsibilities. Programming components include:
PART 1
  • How to be an educated consumer related to setting up home-based services
  • The caregiver role—knowing what to expect
PART 2
  • Medication management, diet & other health factors to monitor
  • Caregiver networks of support
The Brookdale Center for Healthy Aging & Rehabilitation is a 54-bed acute rehabilitation hospital, located at NCH’s North Naples Hospital campus, serving Collier County and the surrounding area.
