2023 Nurses Week

We had an amazing turnout at the Nursing Excellence Awards Ceremony held in the Grey Oaks Country Club, Naples. Everyone had an opportunity to network, celebrate and get inspired! For the first time, the Nurses Week Committee members elected to have two staff nurses serve as the master of ceremonies for our 2023 NCH Nursing Excellence Awards. 

Julie Lee, RN/ NCH Nursing Registry and Anthony Silva, RN/ NCH Physician Group Surgical Asst. did a great job keeping the audience entertained.

Happy last week of Nurse’s Month (May) and almost Memorial Day!

Many of you have been wrapping up school activities with your children or even yourselves. I have attended my oldest niece’s college graduation from FSU and my youngest niece’s 8th grade graduation (I am a proud Aunt) 😊.

Sometimes it seems like time is flying by. I hope that you have been able to spend some time doing what makes you happy! You deserve some happy time to decompress from the stressors of a life in the healthcare profession.

It has been awesome to hear from many of you as Ilia and I round through the hospital and during the “Ice Cream Social” time. We both have ice cream sessions again this week and we look forward to meeting many more of you.

One thing that comes up time and time again while rounding is the cost of living in southwest Florida and the overall increase in inflation that is impacting you on a personal level. We understand your concerns.

Many of you question the pay and use of travel agency staff. We realize that the use of traveler staff is not financially sustainable. We are thankful for the travelers that came and helped us during a very challenging time in healthcare: Covid surge, seasonal support and Epic conversion.

Now, thankfully we are not surging with Covid patients (let’s pray we won’t again), our census is stabilizing (under 500 today) and we go live with Epic in two weeks. This means that our plan will be to allow most travel contracts to expire in June as planned and end other’s contracts the last week in June. This lets us have support for Epic and allow our Super Users to be resources outside of patient care through June.

By phasing the travelers out of staffing by the end of June we will allow our hospital to retain financial stability. This will also allow the hospital to invest in our core staff. We appreciate our core staff for continuing to work at NCH.

I recognize how demoralizing it can be to work along side others (travelers, SSP) that make a more inflated salary. Our goal moving forward is to reserve incentive programs for our core employees, to reward loyalty and promote retention.

You do amazing things every day for our patients, families and teams. We hear you and understand what you would like to see happen with salaries. Starting with the traveler reduction is the first step in the process.

As I wrap up this update, I again want to thank you for the great work that you are doing.

It’s a tough time to be in healthcare but I truly believe that we have the best team. We will end the summer and start the fall welcoming new nurses and other new team members. We will continue to build on the foundation that we are ONE Team, STRONGER together. Team, it’s not just a slogan, when the Department of Nursing works as ONE team and supports each other, we will overcome any challenge and promote the profession of nursing!

Happy Monday after Earth Day and last week of April!

It’s a busy time of year as we prepare for Epic, while handling life and (if you have kids in school), all the end of the school year activities. My oldest niece, Annamarie, is graduating from FSU on Friday (I am Tallahassee bound on Friday) and my youngest niece, Mary, is graduating from Middle School in May. I am definitely a proud Aunt!

I always like to start my updates reflecting on things that bring me joy, for me it’s my nieces and nephews. I hope that you can take a minute to reflect on the joy in your life. Even reflecting for a couple of moments is good for our well-being.

We are so inclined by the nature of our work to push our well-being to the back seat. Let me be the reminder that you must take some quiet time for yourself. Always remember how much you are needed by your team, by friends and family, by your pets 😊, the list could go on and on.

I also want to relay how proud I am of the work that you do every day and I am sincerely grateful for each of you. We are ONE team and stronger TOGETHER!

Remember that when interacting with your peers and with other departments. How much better would our lives be if we were supportive of each other every day, no mater the situation? Have grace when others make mistakes. It is sometimes easier said than done, but I really believe that we can impact our environment by just supporting each other.

We are ONE sisterhood or brotherhood in nursing!

As we come to the close of another very busy week, I want to take a moment to wish you a restful Easter holiday. For those who are working on Easter Sunday, thank you for being her with our patients.

Here are a few recent updates:

  • EPIC
    • We have officially started training for EPIC go-live. Great job completing your prerequisites! Remember to arrive early to your training class.
  • Visitation Policy
    • The COVID specific visitation policy has been retired. Our general patient visitation rights policy is in effect. This policy is intended to allow flexibility to support our patients while allowing clinical judgment to limit visiting when necessary.
  • Nursing Excellence
    • On April 7, we launched our nursing excellence survey being administered by Tipton Health Communications. The survey is intended to assess our nursing structures that are linked to clinical excellence so that we can address the gaps identified in order to improve your work environments. The survey will end on April 22 and we will share the results with you shortly after that time.
    • We recently implemented a department-wide “Nursing Engagement Council” being led by Nicole Bishop Tregea, Administrative Director of Inpatient Services at the North Naples Hospital. The purpose of the council is drive initiatives to support your well-being. The council has already held their first meeting and have drafted a calendar of events for fun things to come. More to come on that.
  • Award Nominees
    • Congratulations to all of the Nursing Excellence Award Nominees! We recognize and celebrate you. We will be celebrating Nurses Week May 6- May 13. The calendar of events for that week will be released on Monday, April 18.
  • Operations
    • This week, we closed the 3 Central and 3 SouthCentral units at the Baker campus to begin preparing the units for construction. Patients  who would have normally been admitted to those units will be placed in other units as appropriate.
    • IV pumps are currently being upgraded in preparation for EPIC. Thank you for your attention to this process and your patience in getting this completed.
  • CNE/CNO Sessions
    • We have started holding our “Ice Cream with Ilia and Gina” sessions. The suggestion for these sessions was given to me by Noam Goldberg on the 6th Floor North Naples Hospital
    • The sessions are going great and we are getting excellent ideas from you! Thank you. Remember that these sessions are intended to personally connect with you and gather ideas for how we can make our hospital the best place to work. We will follow-up with you on ideas to be implemented as a result of your feedback.
  • Housing

Keep up the great work and continue to support each other. We are stronger together!

Past System Updates

EPIC

We are two weeks away from go-live.

Please get to training if you haven’t already and don’t forget to complete your testing.

For everyone that has completed training, THANK YOU! Practice in the playground to get more comfortable with the system. You will not be able to work without training so don’t short change yourself, get to training and get exposed to an awesome medical record program.

Keep checking emails, as we get closer to go live more information will follow about what’s going to happen during the transition on June 4!

Thrombectomy Capable Stroke Survey

The Joint Commission was here last week and they were so impressed with our Stroke Program. They confirmed our program’s certification and encouraged us to go to the next level of certification.

This happened because of the great care that you give to these patients. All areas of the hospital were recognized for the part they play in stroke care. Thank you!

Documentation

It is critical to document the great care that you give.

Pain assessments and follow up, restraint documentation, IPOC’s (care plans) may seem minor but they document the very important care that you are giving, without this documentation it appears to those that monitor hospitals (The Joint Commission, AHCA) that we just didn’t provide the care to our patients.

Protect our patients and yourself by documenting. On a positive note, Epic will make it easier over time.

Helping Hands

Thanks to everyone that has been picking up shifts. You are so needed and I want you to know how much we appreciate the help.

Every hour that you work is an hour that a direct care nurse/tech/US/PCA/Sitter (many more) has to spend with their patient.

Flexible Staffing/Shifts

There may be times that you are available for a partial shift to assist with patient care. Let us know.

I would much rather have support for our patient for 4, 6, 8 hours than have no support for 12.

Please contact staffing if you are interested or talk with your leader directly who can also help.

The time is near: June 4 is go live!

Please attend your training and attend any practice session that is available. We practice in the “Playground.” Once you have completed your training session, you will learn all about the “Playground” environment.  😊

Nurses Week

What a fun week!

Make sure you see the calendar of events and participate if you can. The mini horses will be back at both campuses. Pet therapy really is therapeutic.

I actually paid money to visit the Cat Café in Fort Myers. It was wonderful!

Your input on all things that impact your nursing practice is so important. You know what will work, please share your knowledge.

Recruitment

Remember what I said about the end of the school year? This includes nursing schools.  Yeah for us! 

We need these new nurses to come and work for NCH but most of all, we need them to like working here. Please support, train and mentor our new to practice nurses.

Let’s put that terrible saying, “Nurses eat their young,” to rest forever. It’s our professional duty to prepare the next generation of nurses. 

Let’s do it the right way, you have so much to offer!  Your experience alone makes you a hero to these new grads.  😊

Ice Cream

Ilia and I are continuing our Ice Cream Socials. Tonight I am at the Baker ED, CCU and 6S. Every area will have time scheduled. Some may have a coffee and doughnut time if your hours require an early morning visit. I won’t make you eat ice cream for breakfast!

Leadership Update

We are beginning to fill some of the open leader positions. Most notably, the assistant director position. This position was developed to support you in the evening and early night shift hours.

Michelle Hoffman, the Baker OR/CVOR Director, will be stepping in to a CVOR RN position. We support her 100% and thank her for her time as director. We do have an interim leader who will fill in as director starting in May. More information soon.

Recruitment and Retention Statement from Your Leaders

Dear Nursing Colleagues,

During leadership rounds, we have received several questions regarding NCH’s plans to address our workforce’s recruitment and retention.

We wanted to take some time to update you on what is being done.

The leadership team is in constant review and discussion of workforce needs at NCH, in the state of Florida, and nationally. The challenges we are experiencing at NCH are not exclusive to our organization. Many hospitals and non-healthcare industries around the country have been impacted by inflation, housing cost surges, and workforce shortages.

The annual U.S. inflation rate is 8.5% for the 12 months ending March 2022, the highest since December 1981, according to U.S. Labor Department data published April 12. Elevated inflation has been driven by supply chain disruptions and pent-up consumer demand for goods post- Covid-19. Nationwide, home prices have also been affected, with F.L. houses rising 18.8% year over year in 2021.

In addition to this, with 10.9 million job openings, the country is experiencing a labor shortage across all work sectors, including healthcare. The U.S. Bureau of Labor Statistics projects 194,500 average annual openings for registered nurses between 2020 and 2030, with employment projected to grow by 9%.

On a positive note, history has taught us that inflation and unemployment challenges, as mentioned above, do resolve with time. Currently, unemployment rates were lower in March in 37 states and stable in 13 states and the District of Columbia. All 50 states and the District had jobless rate decreases from a year earlier. The national unemployment rate, 3.6 percent, declined by 0.2 percentage points over the month and was 2.4 points lower than in March 2021, the U.S. Bureau of Labor Statistics reported this month.

This is truly an unprecedented time in our economy and has prompted action at the government level. Locally at NCH, we meet as a leadership team weekly to discuss the impact of the economy on our organization and workforce. We are following the economic state and legislative responses closely and working internally to address NCH needs.

Please realize that there is no quick or easy fix to a comprehensive systemic problem. There are, however, several strategies that have been instituted to mitigate recruitment and retention.

Below are some action items implemented:

Recruitment and Retention Efforts at NCH

Recruitment

  • Highlighting critical-to-fill roles on indeed.com as priority postings. Positions are placed at the top of the page.
  • Highlighting specialty roles on professional organization sites. Paid advertisements to highlight positions.
  • Offered several Hiring Fairs- 2021: System-wide hiring event, 6/19/21; 2022: Virtual hiring events, 2/15, 2/16, 2/17, and two face-to-face hiring fairs scheduled this quarter, 4/28 and 4/30.
  • Offering sign-on bonuses for specialty and critical-to-fill roles. Examples below, but not inclusive.
    Clinical Techs $5K
    • PACU RN 10K
    • R.N. med/surg 10-20K (shift and location dependent)
    • Respiratory 20K
    • L&D 20K
  • Offering referral bonus ($2,000 for non-clinical staff, $5,000 for clinical staff) for employees who refer a new hire. The employee has to be F.T. and has to complete the 90-probation period.
  • Conducting High School and Post-Secondary Schools Outreach: NCH nursing and recruitment teams visited Golden Gate & Naples high schools (Feb) and several University nursing graduating classes (FGCU) to offer a lecture on working at NCH. We are continuing to partner with high schools and Universities to provide talks on working at NCH.
  • Implemented the Graduate Nurse Program for students completing the nursing program who are not licensed yet so they can begin working right away.
  • Hired a new Housing Coordinator to assist with new staff placement and work with community partners on housing options.
  • Started fellowship groups in Critical Care, OR, Emergency Department, L&D, and Mom Baby and have instituted ongoing fellowship start dates for many areas.
  • Instituted Clinical Tech Work-Study Program with Lorenzo Walker to grow this workforce.
  • Partnered with travel agencies to onboard travel nurses to supplement staffing.
  • Supplemental Seasonal Program- Onboarded SSPs to assist with seasonal staffing.

Retention

  • Instituted the Recruitment and Retention Workstreams- These are five workgroups that meet to evaluate current recruitment and retention programs and generate new ideas for both. Some ideas are currently under cost analysis by our finance team.
  • Conducted a housing survey and recruitment survey to gauge employee housing needs/interests.
  • Partnered with Collier County Housing Coalition to discuss housing needs and options. We have two NCH leader representatives on the coalition.
  • Conducted a comprehensive market analysis of various roles to assess compensation against benchmark:
    • SPD, OR, E.D., Respiratory, Clinical and surgical techs
    • R.N.s
    • School Health Aides
    • School Nurses
  • Offered two adjustments in 2021 (May and October) for affected roles.
  • Continue to offer extra shift incentives for additional shifts during the pandemic and in-season.
  • Instituted the Helping Hands Program to allow non-clinical and clinical employees to help provide care outside of their primary work environments.
  • Continue the PTO Cash-out Program (6/22-6/29/2021).
  • Instituted the “Create your Culture” program to gain ideas from employees on how to improve our work environment.
  • Continue to offer free health insurance.

Please be patient as we work through this time. There is not an easy fix, but we are evaluating our workforce needs and solutions from various vantage points. We appreciate and value each of you and thank you for all that you do for our patients every day. Thank you.

The shared Governance Central Councils wanted to share their up-and-coming project, and they are inviting you to be a part of the fun. Use your voice to impact the future of NCH and have fun while doing it!

Teaching and learning have partnered up with the leadership team, quality and zero harm. They are helping to trial the “Call Don’t Fall” initiative on two units. They will be wrapping up the trail in just a few weeks.

Once we have the results, we will share them with you. We are looking for others who may like to help initiate this move forward across the organization. If you are interested, let us know.

Professional Practice and Development (PPD) are looking to increase your learning resources. They are partnering with the simulation lab to work on offering skills advancement in IV starting and NG tubes. They are also product testing new items. Be on the lookout for this information.

Nursing Excellence Committee (NEC) is working on quiet at night for our patients. Sleep is needed for proper healing and the NEC team is passionate about this project

What is the Surgical Technician Clinical Ladder?

The STRONG program embraces the belief that all surgical technologists are responsible for their professional growth and development. The program goals are: • Promotion of self-accountability and accountability to the patient and the team • Improve quality of care and patient outcomes • Reward excellence • Improve visibility and acknowledgment of the surgical technologist role 4 The STRONG committee is responsible for the design, revision and management of the Clinical Ladder.
